CAREpath Partners with MedChart for Collection and Access to Medical Records
With MedChart’s automated acquisition process, CAREpath can seamlessly obtain medical records online from all of their patients’ healthcare providers and have constant access to them via an online portal. Access to the records can be shared with CAREpath patients and their families.
“This highly encrypted program has streamlined patient care and brought record keeping into a whole new era,” says Sherry Hnatyshyn-Webster, RN, Director, CAREpath. “Our patients, their families and their caregivers now have unprecedented access to all the data necessary to make decisions that will improve the efficiency and accuracy of their treatment and save valuable time.”
Created to empower patients to access and review their medical records, MedChart is a secure cloud-based patient portal which provides patients with their official health records at their fingertips.
MedChart collects records from all providers in Canada including general practitioners, specialists, dentists, pharmacies and naturopaths. All patient data and information, including electronic and paper records, imaging, test results and physician notes are organized and transferred digitally via the secure MedChart cloud-based portal. Records are searchable by date of visit, type of visit and type of procedure and are delivered as they appear in the health information custodian’s files.
“CAREpath is embracing patient empowerment and prioritizing placing patients at the center of their care,” says James Bateman, CEO and co-founder, MedChart. “They are genuinely dedicated to the wellbeing of their patients and MedChart is very excited to be a catalyst enabling the patient access to information so they may be a true partner in their care.”
MedChart (www.medchart.ca) is empowering individuals to acquire, manage and share their medical information online across North America. Founded at the University of Toronto, we are transforming the healthcare system by placing the patient at the center, lowering barriers for patient-access, and providing a transformative approach to universal health data interoperability.
CAREpath (www.carepath.ca) is a unique healthcare navigation program serving more than 900,000 Canadians across the country. For the past 13 years, its team of highly trained care specialists that includes physicians, nurses and support staff, has helped patients maneuver through the healthcare system to get the right kind of care, at the right time and in the right place.
